What is this service?

If you’re looking for support during or after a cancer diagnosis we’re here to help you navigate your way through – what sometimes feels like a confusing world – to access a wealth of local health and social services.

Our Macmillan Rehabilitation and Support team is run in partnership with local healthcare organisations and Macmillan. It’s delivered across Bristol, North Somerset and South Gloucestershire – and covers all cancer diagnoses.

What does this service offer?

The story of cancer is changing. People with cancer are living longer after their diagnosis than they did 40 years ago. Cancer is increasingly about living with cancer, and there are many people experiencing issues that require support several years after an initial diagnosis and treatment.

We offer a range of services, including:

  • A full assessment of your needs – physical, emotional, social and functional (daily living activities such as washing, dressing, cooking, shopping, working and accessing community services etc)
  • One-to-one support in your home
  • Access to a numerous health and wellbeing events and support groups.

You might be looking for support to manage fatigue, your home, return to work, improve low mood and/or address physical changes. Whatever it is, we’re here to help.

Who is the service for?

This service is for anyone looking for support during or after a cancer diagnosis. You must be:

  • over 18 years of age
  • registered with a GP practice in Bristol, North Somerset and South Gloucestershire
  • consenting to a referral
  • not deemed as end stage, requiring palliative care, or have a prognosis of less than 6 months.
