What is this service?
The Bristol Community Health Community Discharge Coordination Centre (CDCC) is the single point of exit for all hospital discharges into community rehabilitation services. The service is made up of trained clinicians including physiotherapists, occupational therapists and nurses.
We also accept referrals for patients in the community who would benefit from a rehabilitation service.
These community rehabilitation services include: rehab centres, rehab teams and community therapy beds.
What does this service offer?
We accept referrals from any health or social care practitioner. The referral is handed to a case manager who will triage the referral and arrange to complete an holistic assessment. The assessment can take place in hospital or in the community. Following the assessment the case manager will agree a patient centred rehab plan specific to the person’s needs.
If the assessment is completed in hospital, the case manager will work together with the patient, family and hospital staff to develop a rehabilitation plan to facilitate a safe and timely discharge. This discharge might be back to the patient’s own home with support from a community rehab team or to a community rehab centre or community therapy bed located within a care home. If the assessment is completed in the community, the case-manager will refer you onto the most appropriate service to meet your needs and to support you to live life well at home. We can also support with arranging for equipment to be provided for those patients who need some support at home.

Who is this service for?
Patients must adhere to the following criteria:
- aged 18 or over,
- registered with a Bristol GP and a Bristol resident,
- requires multi-disciplinary rehabilitation service in the community. Please note that rehabilitation may take place in a number of settings, including rehabilitation centres (provide residential recovery and rehabilitation support for short periods), community rehab beds (provide nursing and some therapy support within a nursing home care setting), or via rehabilitation teams (provide rehabilitation in the patient’s place of residence).
How can I access this service?
You cannot access this service yourself; referrals must be completed by a health or social care practitioner.

To refer a patient, call 0117 342 6667. Phone lines will be answered 8am – 5pm, Monday to Friday and 8am 4pm on Saturdays. During out of hours or busy periods, a voicemail service will be available. We will always acknowledge receipt of messages or referrals by the end of each working day.
