What is this service?
Our dedicated community nursing teams provide healthcare and support to housebound patients in the home.
The teams work in partnership with local healthcare professionals – for example GPs and other specialist Bristol Community Health services – to improve the health and independence of local patients.
By actively supporting those with long term conditions to manage their conditions in the comfort of their own home, our community nurses help to prevent hospital admissions.
Community nursing is our largest service, with staff based in teams all over Bristol.
What does this service offer?
Community nursing teams provide a wide range of care and support to housebound and temporarily housebound patients who are over 18. This includes:
- Coordinating care for patients with long-term conditions or complex health needs.
- Administering medication.
- Dressing and managing wounds.
- Caring for patients after surgery.
- Supporting issues around continence.
- Caring at end of life.
They’re a team of healthcare professionals, including district nurses, community nurses, healthcare assistants, assistant practitioners, and advanced practitioners.
They provide short-term interventions or episodes of care for housebound and temporarily housebound patients, who are over the age of 18, registered with a Bristol GP and have a nursing need. They work closely with GPs and other community health services to prevent admissions to hospital and care homes. They also work with patients, and their families and carers, to equip them with the skills and knowledge to manage their conditions independently at home.
Out of hours nurses provide care between 8pm and 8am and at weekends. Please note, this service works closely with our Rapid Response (urgent care) Teams. Read about a night in the life of an out of hours nurse.

Who is the service for?
The service is available to housebound patients and temporarily housebound aged 18 and over who are registered with a GP surgery in Bristol. This includes patients who live at home, in supported housing or in a residential home. Patients who live in nursing homes are not eligible because their nursing care is already provided.
How and when can I access this service?
To access this service you must be referred by your GP or another healthcare professional.

Patients are visited on a weekday between 8am and 7pm. During evenings, nights and weekends, our out of hours nurses are available to deliver urgent care that cannot wait for a day time appointment.
Out of hours nurses provide care between 7pm and 8am, 7 days a week. Please note, this service works closely with our Rapid Response (urgent care) Teams.
