What is this service?

Intermediate care within Bristol includes a range of services that Bristol Community Health provides in partnership with Bristol City Council’s Health and Social Care Team. Intermediate care includes rapid response, community rehabilitation and re-ablement, residential centres, and hospital based teams.

What does this service offer?

Our intermediate care services include urgent or planned healthcare assessments for people in their own homes or in residential home; support can also be provided when a patient leaves hospital to return home. The teams are made up of specialist practitioners who aim to help you maintain your health and live as independently as possible.

Intermediate care services include the following:

Rapid Response, who offer care for those who are suffering from acute episodes of illness. We will visit patients in their own home or place of residence for up to 7 days.

Community rehabilitation and re-ablement, who support patients in their own home or place of residence.

Residential centres provide recovery and rehabilitation support for short periods of time.

Hospital based teams provide a link with our Intermediate Care Services in the community and help to facilitate the discharge of patients from hospital care.

Who is the service for?

This service is for people aged over 18 years of age. We may also accept referrals for people aged 16-17 by exception.

How can I access this service?

Intermediate care is not a service to which you can refer yourself. If you are a healthcare professional and wish to refer, see below.

Are you a healthcare professional who wants to refer to this service?

There are two ways for GPs, health and social care professionals and hospital staff* (*hospital staff can refer to our re-ablement, rehabilitation and to facilitate hospital discharge or prevention of admission to long term care) to refer to our Intermediate Care services:

Rapid Response Referrals

We accept telephone referral only from GPs, and health and social care professionals.

To refer call our Single Point of Access (SPA) on 0117 903 0202. This telephone line is open 24 hours a day, 7 days a week.

Planned referrals for re-ablement, rehabilitation, or to facilitate hospital discharge or prevention of admission to long term care;

  • referrals are accepted from GPs, hospital staff and other health and social care professionals via telephone or faxed referral forms.
  • referral line 0117 342 6667 between Monday and Friday 8am to 5pm (answerphone available outside office hours).
