What is this service?
Our Community Nurses for Older People (CNOPs) are registered Nurses who work with older people to promote good health and spot early deterioration of conditions which affect patients’ health and well being. They will visit patients in their own home or place of residence for a limited period of time.
Where necessary, our Community Nurses for Older People·will work closely with our other services to ensure each individual patient can address their own health care needs and they will find support for those who find this difficult.

Who is this service for?
This service is for older people who:

wish to live independently at home for longer
-
need support to prevent admission to hospital or long term care
-
need education and support for long term conditions to enable them to lead healthier lives
-
need signposting and support to access other services
-
need support to access the services they need
Older people and their carers who:

require broad health needs assessment wherever possible in their own homes. This may include an assessment of health and social care needs, a falls assessment, continence assessment, carers’ assessment and/or mental health awareness and support.
How can I access this service?
You must be referred by your GP into our Community Healthcare Teams. Click here for more information on these teams.
