What is the service?
Our Community Matrons have extended clinical knowledge and skills. They aim to help patients manage their health problems and in doing so prevent unnecessary admissions to hospital to enable them to live as independently as possible.

Who is this service for?
This service is for adults with a long term condition e.g. Chronic Obstructive Pulmonary Disease (COPD) or Heart Failure, who require different levels of support. This may include:

managing an acute episode of a long term condition

helping patients and carers to develop plans to maximise patient independence

liaising with specialist services to co-ordinate an effective approach to care

liaising with hospitals to enable fast and effective discharge
How can I access this service?
You must be referred by your GP into our Community Healthcare Teams. Click here for more information on these teams.
