What is the service?

Our Domiciliary Physiotherapy service provides rehabilitation and self management strategies for patients within their own homes or in a community setting. We aim to help people to recover their movement and function as much as possible through teaching and helping patients to engage in movement, exercise and therapy techniques.

What does this service do?

Our specialist physiotherapy teams deliver expert advice and support to patients. We will help to manage individual patient care plans in the most appropriate way so as to deliver maximum benefit to the patient based on up to date evidence based treatment guidelines.

Who is this service for?

People who need to receive physiotherapy in their own home or place of residence and have one or more of a wide range of conditions including:

neurological conditions
respiratory conditions
patients who have had a fall

You must be referred into this service. Referrals are accepted from all healthcare professionals. Completed referral forms can be faxed to us on·the number below.
