Depending on a patients individual need, the appropriate health care professional from our team will visit the patient at their home or in their place of residence.
What does this service do?
The different health care professionals in our Community Healthcare Teams each deliver care in their individual area of clinical expertise. Patients being seen by our Community Healthcare Teams are likely to be visited on a weekday between 8am and 8pm. We also operate an out of hours service during evenings, nights and weekends.
Where necessary, our staff work in partnership with other health care professionals who may be involved in a patient's care, such as GPs, specialists, Health and Social Care professionals, hospital and voluntary sector staff.

How can I access this service?
To access this service you must be referred by your GP.
How can I contact this service?
Patients who have been referred into the care of our Community Healthcare Teams can make contact with us and should be provided with a contact number from the health care professional that visits them.
We operate from multiple locations across Bristol. Your local GP Practice should be able to give you the telephone number for your local Community Healthcare Team.
