What is this Telehealth?

Telehealth is a remote monitoring device available to people with COPD or Heart Failure. It supports people to understand their symptoms and increases people's confidence to be more self sufficient. The Telehealth service is monitored by a persons named clinician. It is not an emergency service.

What does Telehealth do?

Once you are referred into Telehealth a member of the Telehealth support team will come to your home and show you how to use the device. Once you feel happy to use the device you will be expected to make daily recordings. The recordings you make will be about how you are feeling, you may also be expected to take your weight and blood pressure. All of the information is sent to your named clinician, through a secure IT network.

Clinicans will look at the daily recordings and this information enables the clinician to understand how well you are monitoring your condition. The clinician may call you to offer support and advice or they may arrange to visit you. Telehealth is a very useful tool to support you to understand your condition better and improve your confidence to take control of your health.

Who is Telehealth for?

If you are an adult with COPD or Heart Failure and feel this device would really help you manage your health better; talk to your clinician about Telehealth.

How can I access this service?

You must be referred by your clinician. How can I contact the Telehealth support team?

The Telehealth support team is available to set up units between Monday and Friday. Clinicans will also be available to monitor your recordings on from Monday - Friday. Please note that Telehealth is not an emergency response service. The Telehealth support team are based at:
