What is this service?
We are working in partnership with local healthcare organisations and Macmillan to deliver the One-to-One Support Service as a two year pilot. This service is aimed at cancer survivors; to help them manage any symptoms they have developed as a result of cancer, in order to improve their quality of life and function.
What does this service do?
We provide support in the community for people living with and beyond cancer. We offer advice and support on a range of issues including:
- Symptoms of pain, breathlessness, decreased mobility and fatigue
- Recognising signs of cancer recurrence and when to get medical help
- Returning to usual activities like work or hobbies
- Getting involved with other support within the community
Who is this service for?
Adults who:
-
are over 18 years of age
-
are registered with a GP Practice within the North, South and West locality of Bristol
-
have a diagnosis of cancer
-
consent to a referral
-
require support to be able to self manage the effects of their cancer as a long term condition
-
are deemed as being a cancer survivor
-
are not deemed as end stage/requiring palliative care
How can I access this service?
We accept referrals from patients themselves, GP’s or other professionals or families / carers; providing the patient meets the referral criteria listed above. You can download a referral form from the left hand side of this page, in the 'Document Downloads' section.
How to contact us
This service is open from Monday - Thursday, 8am - 5pm. We are based at Shirehampton Health Centre but can visit patients in their own homes.
