What is this service?
The Bristol Community Health Palliative Care Home Support Service (PCHS), work with other healthcare professionals to offer care and support at the end of life. The service is led by clinicians with many years of experience in caring for people at this time of life. The team is based as Avonmouth but care is carried out in patient homes. What does this service offer?
PCHS provides personal care and emotional support for patients who wish to be at home as they approach the end of their lives. This care includes personal care and emotional support for patients as well as family members, to meet individual patient needs.
Our experienced nurses and healthcare assistants are able to increase visits dependent on the patients individual requirements. Our team also works alongside other healthcare professionals who may also be providing care, as well as GPs and hospice nurses to ensure a coordinated care approach.

Who is the service for?
We can offer care to any adult patient at the end of their lives who wish to be cared for in their own home. Patients must be registered with a Bristol or South Gloucestershire GP. Care for our patients continues every day of the year, with visits scheduled between 9am and 9pm.
