What is this service?

The Bristol Community Health Wound Care Service supports patients with non-healing or slow to heal wounds. Our specialist nurses support Bristol communities, and healthcare staff, with any type of wound that requires advanced medical attention, including trauma wounds, surgical wounds, leg ulcers and pressure ulcers.
Our team also runs a pressure ulcer prevention project in order to advise and work with patients to prevent pressure ulcers developing.

What does this service offer?

Our service can provide a comprehensive assessment if you have a non-healing or slow to heal wound, that requires advanced wound therapy treatments.

As assessment can take place in your home, with your practice nurse, or within a clinic.

After your assessment, a treatment plan will be drawn up and shared with your practice nurse or community nurse, who will then undertake the care. Depending on this plan, we may arrange to visit you to review the care to ensure that the wound is progressing as expected.
If you have wounds or swelling on the lower limb, a Doppler assessment can be undertaken to assess the circulation supply and to consider the use of compression bandaging. A number of advanced wound treatments can then be considered as necessary.
Our service means you will continue to see your regular nurse as usual, but with expert assessment and guidance from our team.

Who is the service for?

Our service will see patients who are over 16, registered with a Bristol based GP, and under the care of a community nurse, except in exceptional circumstances.
