We support over 300 patients to ensure that their wishes are met

Many of our services support patients to leave hospital quickly and manage their condition in the comfort of their own home.

Here at Bristol Community Health, we also support patients who are approaching the end of their lives. Our Palliative Care Home Support Service (PCHS) provides nursing and personal care for patients who wish to be at home at this stage of their lives.

This years 'Dying Matters' week, aimed at encouraging people to consider their wishes for their own end of life, starts today, 12 May. Our PCHS Team took the opportunity to share more about what they do and how the support they offer.

"Palliative Care and end of life care, is often a difficult and sensitive subject to discuss. Within our team, we make it our goal to ensure that those who receive care from us, as well as their families and carers, feel comfortable and reassured that we are providing the best quality care possible."

"We support over 300 patients a year to ensure that their wishes are met at the end of their lives.. We have a team of 18 staff, comprised of registered nurses and healtch care assistants who are trained and experienced to support people well."

"However, we can't provide the care we give alone, and we know that a combined approach to providing care and support works best. Our partners include other teams within Bristol Community Health, as well as external end of life care providers such as Marie Curie, St Peters Hospice and GPs. We ensure that the patient is at the heart of the care we are providing, and that at whenever possible the patients' wishes are met."

"As a team, we know we are really are making a difference to people's lives at this really important time. Palliative care is not just about dying, it's about helping people have the best quality of life in the time they have left. We understand the importance of getting is right, and do our utmost for everyone who needs this support from our team".
