We are proud to be working in partnership with Macmillan Cancer Support and local healthcare organisations University Hospitals Bristol; North Bristol Trust; and South Gloucestershire Community Healthcare services, to implement and deliver a One-to-One Support Service for Cancer Patients as a two year pilot.
The service at Bristol Community Health, which is funded by Macmillan, is staffed by a Community Nurse and a Complex Case Manager who can give support in the community for people living with and beyond cancer. The team can offer support for managing symptoms, help to return to usual activities like work or hobbies, and advice for recognising the symptoms of cancer recurrence and when to seek medical help.
This service is aimed at adults living with or beyond cancer, not for patients who are at end stage/palliative care - these patients can be referred into our Palliative Care Home Support Service, to find out more about this service, click here.
Leanne Hall, Complex Case Manager at Bristol Community Health, said: "When someone is living with a cancer diagnosis, they can have many support and information needs and it can be difficult to know where to turn for this additional help. We recognise the importance of assessing and identifying these individual needs. Through the Macmillan one-to-one support service we will be working with doctors, nurses and other healthcare professionals such as physiotherapists and occupational therapists across Bristol, to offer additional support, information and signposting for people as they move forward following their cancer diagnosis and treatment."
Maggie Crowe, Macmillan Development Manager, added: " Cancer is the toughest fight that many of us can face so it is important that cancer patients are given the right support at the right time. The overall aim of the Macmillan one-to-one support project is to create a workforce to deliver the best care for people living with and beyond cancer."
Within the pilot period of our One-to-One Support Service, only referrals received for patients who are registered with GP surgeries in the North and West of Bristol can be accepted.
