What is the service?
We provide specialist community based services for adults with learning difficulties. Through these services we help to promote equal access to mainstream health services. Our service consists of three teams covering North, South and Central Bristol.
We operate an open referral system, which means that we will accept referrals from health and social care professionals, carers, and service users themselves. We will carry out a service eligibility assessment if required.
What does this service do?
We work with adults with learning difficulties, their families and carers. Our teams are made up of the following healthcare professionals:

Specialist Intensive Response Nurses

Speech and Language Therapists
Our staff work together with Psychiatrists and Social Workers to provide the best possible care for our service users.
We provide specialist therapy for service users in the setting most suitable to them. This could be their home, workplace or a hospital.
We help to improve people's health and wellbeing by providing support with:

accessing GP health checks and help with health action plans

other services available to people with learning difficulties, for example advice and training

complex health needs, for example eating and drinking difficulties and challenging behaviours

healthier lifestyles, for example diet and activities
-
helping people to fulfil their lives, for example with how they spend their time and helping them to become ready for work

expressing feelings, managing anxiety and improving mental wellbeing
Who is the service for?
This service is for adults who have a learning difficulty and are registered with a Bristol GP. A limited service may also be offered to children. To find out more about this please contact us.
How can I access this service?
Access to our service is by referral only. Anyone can refer using the referral form in the 'Document Downloads' section on the left of this page. Please ensure you select the correct referral form for your area of Bristol (North, Central or South).
We may need to meet people new to the service to check that we are the right service for them. We will contact you to let you know if we need to do this.
How to contact us
Our offices are staffed from Monday - Friday between 9am and 5pm. Our clinicians will work outside these hours when necessary.
