Disabled Adults Resource Team (DART)

What is this service?

We work with disabled adults who primarily have physical impairments. Our team consists of:

Occupational Therapists
Physiotherapists
Psychologist
Speech and Language Therapists
Rehabilitation Assistants
Team Secretaries

What does this service do?

We can work with you on any aspect of your health and the daily activities that are most important to you. We can see you in your home or at another address if you wish.

Our service offers an assessment and discussion of your current needs, and then treatment if that is helpful. We may also advise on suitable adaptations and equipment that could help you.

We will work with you for as long as it takes to reach achievable targets, and let you know how to get back in touch with us in the future should you need to.

Who is this service for?

The service is for adults who:

have on going physical impairments
are registered with a Bristol GP practice
need a team approach

How can I access this service?

People can either refer themselves to DART, or ask others to refer them. If you are completing the form on someones behalf, you must check that they consent to the referral.

We can also accept referrals over the phone. To do this just call us on 0117 961 7165.

We prefer written referrals using our referral form and with as much detail as possible. This helps us to decide if we are the correct team for the person being referred and when we should see that person. You can download a copy of our referral form from the 'Document Downloads' area on the left of this page.

How to contact us

Someone is usually available to take telephone calls on week days between 9am and 5pm. At times there may be an answerphone service and messages may not be picked up until the next working day.
