What is the service?
Our Community Nursing service is made up of registered nurses who assess, plan and deliver care that matches patients’ individual needs. This may include wound care, continence assessment and care, end of life care and giving medication. Our Healthcare Assistants (HCAs) support the work of our Community Nurses. What does this service do?
Our Community Nurses provide nursing care for predominantly housebound patients. We promote and support independence and provide care at periods of time when patients are unable to support themselves, such as following a recent discharge from hospital or a period of serious illness.
Who is this service for?
This service is for adults who are:

aged 18 and over and require nursing care
-
predominantly housebound patients and those and unable to attend a clinic or treatment room for their nursing care.
How can I access this service?
You must be referred by your GP into our Community Healthcare Teams. Click here for more information on these teams.
