What is the service?
We run heart failure diagnostic clinics and nurse-led clinics throughout Bristol and South Gloucestershire in a number of community bases.

What does this service do?
At our clinics you will talk to heart specialists about your symptoms and how they are affecting your daily life. Some tests will be run to see if there is a diagnosis of heart failure. An ultrasound scan of the heart may be performed. This scan is called an echo. It is safe and not painful. The specialist will be able to review the results at your appointment and make a diagnosis. If there is a diagnosis of heart failure, a clear explanation will be given to you and a management plan will be developed to enable you to manage your condition. After this appointment you will be contacted by a specialist nurse and they will start to work with you to develop a treatment plan. If the tests results do not show heart failure you will be referred back to your GP.
If you have been referred to our service because you already have a diagnosis of heart failure our team will talk to you about your symptoms, run some tests and review your current management plan.
Who is this service for?
You may be referred to the heart failure service if your GP thinks you may have heart failure (your heart is not pumping properly). You may:

already have a diagnosis of heart failure, but your treatment needs to be reviewed

have heart failure and worsening symptoms

have just been in hospital, advised that you have heart failure, and need to have your treatment plan reviewed
How can I access this service?
To access our Heart Failure service you must be referred by a GP.
