Intermediate Care and Reablement

What is the service?

We work in partnership with Bristol City Council's Health and Social Care team to provide urgent or planned healthcare assessments for people in their own homes or in residential centres. We can also provide a bridge between hospital discharge and a patient's return to their own home or place of residence.

What does this service do?

Our service is made up of four different teams of specialist practitioners working together. We aim to help people maintain their health and live independently as far as possible.

The teams providing care are:

  • Rapid Response – providing care for people suffering acute episodes of illness. We will visit patients in their own home or place of residence for up to 7 days.
  • Community rehabilitation and re-ablement – providing support to patients in their own home or place of residence.
  • Residential centres – providing recovery and rehabilitation support for short periods of time.
  • Hospital based teams – providing a link with our Intermediate Care services in the community and helping to facilitate the discharge of patients from hospital care.

Who is this service for?

This service is for people aged over 18 years of age. We may also accept referrals for people aged 16-17 by exception. Please contact us to discuss your referral if you are unsure.

How can I access this service?

Patients cannot refer themselves to this service. There are two ways for GPs, Health and Social care professionals and hospital staff* (*hospital staff can refer to our re-ablement, rehabilitation and to facilitate hospital discharge or prevention of admission to long term care) to refer to our Intermediate Care services:

Rapid Response Referrals

  • telephone referral only
  • accepted from GPs and Health and Social Care professionals

To refer call our Single Point of Access (SPA) on 0117 903 0202. This telephone line is open 24 hours a day, 7 days a week.

Planned referrals for re-ablement, rehabilitation, or to facilitate hospital discharge or prevention of admission to long term care

  • referrals accepted from GPs, hospital staff and other Health and Social Care professionals via faxed referral forms only. You can download a referral form from the 'Document Downloads' section on the left side of this page.
  • referrals must be through the faxed referral form. This form is available to download from the 'Document Downloads' section on teh left of this page.
