Dementia support

Everyone is a little forgetful now and again, but if memory loss starts to interfere with your daily life then it is important to see your doctor.

At Bristol Community Health we have worked hard to ensure all our services are 'dementia aware'; this means that over 90% of our staff have undertaken dementia training to at least Level 1, and are supported to help identify patients who may be at risk of developing dementia, as well as spotting the signs and symptoms of dementia.

To support staff and promote awareness of dementia we have developed 'Dementia Champions' in our workforce, who have received additional training delivered in collaboration with the Alzheimer's Society, Carers Support Centre and the Red Cross. Our Dementia Champions promote identification, support and advice and care for people with dementia and their carers.

What does this service do?

If you are seen by a member of Bristol Community Health staff in any of our services, and they feel you might be at risk of developing dementia, or have concerns about your memory, they may:

  • offer you a brief memory test that only takes a few minutes (or refer to our community nursing team to offer you this);
  • give you the leaflet 'Worried about your memory?' for you to read;
  • with your consent, make a referral to your GP for a more detailed memory assessment and/or to discuss how best to support you;
  • offer you a dementia support pack containing information about organisations that can offer help and advice; and
  • discuss with you and/or your carers about whether they feel they would benefit from a carers' support pack.

How can I access this service?

All of our patients can speak to us about dementia or share concerns about their memory with any Bristol Community Health staff involved in their care.
