REACT In-Reach

What is this service?

The Bristol Community Health REACT (Rapid Assessment Emergency Care Team) In-Reach Teams are based in the emergency departments and related areas of North Bristol Trust and University Hospitals Bristol.

The team is made up of nurses, physiotherapists and occupational therapists.

What does this service offer?

Our service is based in emergency departments to support you to return home when you no longer need to stay in hospital. Our teams can carry out an individual assessment of your needs and if necessary signpost you on to the most appropriate service to meet your needs in the community. These may include physiotherapy, GP monitoring, community nursing or social services.

Who is this service for?

Patients who have been in hospital for less than 48 hours due to a current illness or injury that has temporarily affected their ability to manage.

How can I access this service?

You can only access this service if you are within an emergency department.
